Care Manager for Care Management Service

And you’ll get to practice your passion in a non-profit, mission-driven organization that’s known for the highest level of care in our communities ESSENTIAL DUTIES AND RESPONSIBILITIES Acts as a liaison between members and home and community-based service providers to facilitate quality and cost-effective care and outcomes. Provides education and support to members and their families regarding community resources and services. Develop effective, collaborative relationships with key stakeholders including primary care providers, managed care plan providers, home and community-based service providers (HCBS), formal/informal caregivers and families. Maintains regular communication with members through phone calls and home visits to monitor their wellbeing and adjust careplans as needed. Records and documents interventions and member information completely and accurately, in accordance with payer and organization guidelines. Collaborates and communicates with internal team members, including Central Intake and Assessment Team members, to ensure continuity of care for assigned members. Identifies and resolves care plan variances, including barriers to primary and specialized medical care, to ensure effectiveness of member’s careplan. Assesses and addresses member engagement and behavior to support optimal health and functional status. Participates in after-hours on-call rotation requirements, as assigned. Maintains and monitors quality of services through collaboration with the Quality Assurance and Education Team and Care Management Director. Participating in Community Outreach as outlined in Quarterly Outreach plan. Support the training and onboarding of new team members. Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication. Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System, in a professional manner. Performs other duties as necessary to ensure the success of the System. SKILLS AND ABILITIES Communication: Effective verbal and written communication is essential for interacting with clients, families, and other professionals. Empathy and Compassion: Understanding and sharing the feelings of clients, especially those facing challenging situations. Resourcefulness: Knowing about available community resources and how to connect clients with them. Collaboration: Working effectively with other professionals and agencies. Problem-Solving and Critical Thinking: Case managers need to assess situations, identify problems, and develop effective solutions. Organization and Time Management: Managing multiple cases, appointments, and documentation requires strong organizational and time management skills. Cultural Competence: Understanding and respecting diverse cultures is essential for building trust and providing appropriate support. Advocacy: Representing clients' needs and ensuring they have access to necessary services. Ethical and Legal Knowledge: Understanding ethical guidelines and legal frameworks relevant to social work. MINIMUM QUALIFICATIONS Bachelor’s degree in social work or related human services field is required with 2 years of work experience Experience in social work, home and community-based services, healthcare or geriatrics preferred. SUPERVISORY RESPONSIBILITIES LANGUAGE SKILLS Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization. MATHEMATICAL SKILLS Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram format. The ability to deal with problems involving several concrete variables in standardized situations.
