Physician Assistant / Nurse Practitioner – Care Coordination & Specialty Scheduling (Remote)

Description: We are looking for an experienced Physician Assistant (PA), Nurse Practitioner (NP), or highly experienced clinical care coordinator to support a fast-growing healthcare care coordination program. Our team works directly with primary care providers (PCPs) to help their patients navigate specialty care. When a PCP sends a referral, our team contacts the patient, identifies the best specialty care options, and schedules the appointment on the patient’s behalf. Your role will focus on clinical triage, patient outreach, and coordinating specialty care appointments to ensure patients receive timely, appropriate care. Responsibilities: -Review incoming PCP referrals to specialty care -Contact patients to discuss referral needs and preferences -Help identify high-quality specialists and care options -Coordinate and schedule specialty appointments on the patient’s behalf -Communicate with specialty offices to ensure smooth scheduling -Provide patients with clear next steps and appointment details -Escalate urgent clinical situations appropriately -Document interactions and coordination steps clearly Ideal Candidate: -PA, NP, RN, or experienced care coordinator with strong clinical understanding -Background in care coordination, referral management, or patient navigation -Experience working with specialty referrals (cardiology, GI, orthopedics, etc.) -Comfortable communicating with both patients and medical offices -Organized, detail-oriented, and proactive -Experience working with EHR systems and referral workflows -Able to work independently in a remote environment Nice to Have: Experience in value-based care, population health, or care navigation -Background in PCP clinics, care management, or referral departments -Familiarity with insurance networks and specialty access About the Work: -Remote -Flexible hours (part-time or full-time depending on fit) -Ongoing opportunity with a growing healthcare platform -You’ll be helping patients navigate complex healthcare systems and access the right specialists faster -If you have experience helping patients navigate referrals and coordinating specialty care, we’d love to hear from you.
